Eligibility requires meeting Medi-Cal's financial criteria and a county needs assessment. IHSS hours are capped, however, and many families combine or supplement IHSS with private home care to cover the gaps.

How IHSS actually works

IHSS is administered by each county — Sacramento, Placer, and San Diego counties each run their own intake. After a Medi-Cal eligibility determination, a county social worker performs an in-home needs assessment and authorizes a monthly hour allotment for specific tasks: bathing, dressing, meal preparation, domestic services, and protective supervision in some cognitive-impairment cases. The recipient then hires a provider — frequently an adult child or other family member — who enrolls, passes a background check, and is paid the county IHSS wage. It is genuine, meaningful help, and for lower-income seniors it is often the difference between staying home and institutional placement.

The limits families run into

Three constraints surface repeatedly. First, hours: IHSS allotments are task-based and capped, and even maximum awards rarely approach round-the-clock coverage — protective supervision cases top out near 283 hours per month, and most awards are far lower. Second, workforce: the family must find and manage the provider themselves; IHSS is a payment program, not a staffing agency, and there is no backup when a provider cancels.
